1. Download and print the automatic giving form.

2. Decide on the amount you wish to donate by EFT monthly.

3. Fill in the form with bank account information (for checking accounts it is asked that you included a voided check with the form).

4. Sign the form and mail it to the parish office or deliver it in person.
